• New players, U8 and younger:
  • Fill out and sign ALL 3 online forms below.
  • Mail the following to PO box 31181 Tucson, AZ 85751:
    
    • Membership Form.
    • Copy of certified birth certificate. Please note: Baptismal records will not be accepted as proof of date of birth. A copy of a passport will, however, be accepted as proof of date of birth.
    • Payment in the form of cash or check made payable to Fort Lowell Soccer Club, Inc..

• Returning players, U8 and younger:
  • Fill out and sign ALL 3 online forms below.

REFUND POLICY
Refunds will only be granted if requested of the registrar in writing prior to the date of the first scheduled game of the player's age group. After that date, no refunds will be given. No exceptions.
